On 28 January 1986, the space shuttle Challenger exploded just moments into its flight. Later that year, during a crowded session at the annual meeting of the American Association for the Advancement of Science (AAAS), anguished science journalists tried to understand why they had not, in previous years, probed more deeply into the problems of the National Aeronautics and Space Administration (NASA) space shuttle program that led to the explosion.
